I understand and acknowledge that the check-in begins at 10:00 AM and closes promptly at 10:45 AM. As church service begins at 10:30 AM, arriving on time ensures that my child can fully participate in the planned lessons and activities without disruption.

I understand and acknowledge that the use of the facilities at Destiny in the D-Zone involves risks including but are not limited to bodily injury including, but not limited to permanent disability, paralysis, and death. These risks may result from a variety of circumstances including the misuse of equipment or facilities.

I am choosing to use the facilities at Destiny in the D-Zone at my own risk. I understand that my decision to participate in the activities at Destiny in the D-Zone is voluntary. I assume full responsibility for all risks that may arise from using the facilities at Destiny in the D-Zone or from participating in activities at Destiny in the D-Zone. Exempted from this section are any injuries caused by gross negligence or misconduct of any employee/volunteer at Destiny in the D-Zone.

I acknowledge that I have read, know, and agree to all of the policies and procedures relating to the use of the facilities at Destiny in the D-Zone. I agree to comply with all rules, regulations, and policies at Destiny in the D-Zone. I understand the Destiny reserves the right to revoke or terminate my use of the facilities at Destiny in the D-Zone for any violation of rules, regulations, or policies.

I hereby release, waive, discharge, and hold harmless Destiny and all employees/volunteers past or present from any claims, suits, liabilities, judgments, costs and expenses for any property damage, loss due to theft, personal injury or illness, death, or other loss arising from the use of the D-zone.

I acknowledge that I have the skills, qualifications, physical ability to properly and safely use the facilities at Destiny in the D-Zone.

I hereby waive any protections afforded by any statute or law in any jurisdiction whose purpose and/or effect is to provide that a general release shall not extend to claims, material, or otherwise which the person giving the release does not know or suspect to exist at the time of executing the release. I am releasing unknown future claims.

I agree to pay for any and all damages to any property caused by me willfully or otherwise.

I enter into this agreement for myself as well as for my heirs, assigns, and legal representatives.

I understand that I am solely responsible for any medical health, or personal injury costs relating to my use of the D-Zone and its facilities.

I hereby grant Destiny Christian Center International Inc. permission to use my likeness in photographs, video recordings or other digital media (*photo*) in any and all of its publications, including web based publications without payment or any other consideration.

I understand and agree that these materials will become the property of Destiny and will not be returned.

I hereby irrevocably authorize Destiny Christian Center Inc. to edit, alter, copy, exhibit, publish or distribute these images for purposes of publicizing the organization’s programs or for any other lawful purpose. In addition, I waive the right to inspect or approve finished product, including written or electronic copy, wherein my likeness appears. Additionally, I waive any rights to royalties or other compensation arising or related to the use my photo.

I hereby hold harmless and release forever discharge Destiny Christian Center Inc. from claims, demands and causes of action which, I, my heirs, representatives, executors, administrators or any other persons acting on my behalf of my estate have or may have by reason of the authorization.

I HAVE READ AND UNDERSTAND THE ABOVE PHOTO RELEASE. I AFFIRM THAT I AM AT LEAST 18 YEARS OF AGE. I ACCEPT:

I accept that my child may come in contact with someone who has COVID-19 and your child may contract the virus. I also agree that I hold Destiny Christian Center Inc., its staff, and volunteers harmless if my child contracts Covid-19 or any other disease while attending any service or event.

I HAVE READ THIS RELEASE OF LIABILITY AND ASSUMPTION OF RISK AGREEMENT, FULLY UNDERSTAND ITS TERMS, UNDERSTAND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS BY SIGNING IT, AND SIGN IT FREELY AND VOLUNTARILY WITHOUT ANY INDUCEMENT.

By applying my electronic signature to this agreement, I agree that my electronic signature is the legally binding equivalent of my handwritten signature on paper. I will not, at any future time, claim that my electronic signature is not legally binding or enforceable. By electronically signing and submitting this agreement, I 1) acknoweledge that I have read and fully understand the terms of the agreement; 2) voluntarily agree to be bound by this agreement; and 3) certify that I am 18 years of age or older. My signature applies to all pages of this contract. I understand that I will receive a Portable Document Format (PDF) version of this agreement after it is signed at the email address I have provided. To view the PDF document, I understand that I will need software that enables me to receive and access PDF files such as Adobe Reader software or other software capable of reading a PDF file. In order to print and retain a hard copy of this agreement, I understand that I will also need a printer connected to my computer. I understand that if I wish to sign a hard copy of this agreement instead of an electronic version, I must contact the party that requires my signature on this agreement directly.

A parent or legal guardian must sign this agreement on behalf of any minor participant under the age of 18. By electronically signing this agreement, I represent that I have the authority to sign and enter into this agreement on behalf of the minor(s) listed above.
